You can provide great input regarding the services people with developmental disabilities and families in our county and state receive. Funding for programs is dependent on support from state and federal partners and we need your help.

The purpose of this survey is to gain a clearer picture of public knowledge of services and educational opportunities available. There are three versions of the survey, one for self-advocates, one for support persons, and one for community members.

There are no wrong answers. The intent of the survey is simply to assess current knowledge of services and supports, as well as the level of inclusion and quality of life you feel individuals with developmental disabilities experience in your community. Your answers will provide guidance to DDA and to Counties to increase capacity to address barriers and gaps in service delivery in local communities.

This survey should take 5-10 minutes to complete. The last page of the survey will provide useful links to learn more about services and educational opportunities in WA State and in your communities. Please feel free to forward this survey to interested individuals.
